We are seeking qualified candidates to join our team providing access to primary, pediatric, behavioral, dental, and optometry services across the North Country. This position conducts and schedules assessments, referrals, advocacy/supports, and education of patients and enrollees and care team members assuring the patient receives quality services to maintain optimum healthcare needs without barriers.

Requirements: Bachelors of Arts or Science degree with two years of relevant experience; or a License as a Registered Nurse with two years of relevant experience; or a Masters Degree with one year of relevant experience; valid NYS driver's license and reliable vehicle since travel is required; basic knowledge of Microsoft Office; ability to maintain good working relationships with others; strong verbal and written communications skills ; ability to make decisions independently when warranted; high level of professionalism and confidentiality when dealing with healthcare issues.

Two years' experience in human service/social work field, experience in Mental Health, Substance Abuse and medical information preferred.
