Community Health Center of the North Country offers Substance Use Care Coordination which is a free and confidential service available to ALL members of our communities, whether you are a patient at one of our health centers or not. Our Road to Recovery program offers assistance to those at risk of becoming addicted, already addicted, in recovery, and family members of those fighting addictions. We also offer the Healthy Moms & Healthy Babies program which, despite its name, provides support to any parent or guardian (including fathers, grandparents, etc) struggling with drugs or alcohol who are currently pregnant or taking care of a baby or young child.

The Road to Recovery does not need to be taken alone, and CHCNC is here to help.
Adults and children of all ages are welcome to utilize our services, which are offered throughout both St. Lawrence and Franklin counties.

ROAD TO RECOVERY

The Road to Recovery program helps connect people struggling with addiction, as well as their family members, to necessary services such as counseling, chemical dependency, mental health services, employment assistance, support groups, primary care, rehabilitation, food assistance, and transportation.

HEALTHY MOMS & HEALTHY BABIES

Similarly, Healthy Moms & Healthy Babies helps parents or guardians struggling with drugs or alcohol who are pregnant or take care of a baby or young child, connect to services such as care management, recovery peer support, parenting peer support, childcare, education, transportation, other healthcare providers, and can provide support with family court or DSS cases.
