Community Health Center of the North Country offers Substance Use Care Coordination which is a free and confidential service available to ALL members of our communities, whether you are a patient at one of our health centers or not. Our Road to Recovery program offers assistance to those at risk of becoming addicted, already addicted, in recovery, and family members of those fighting addictions. Our Substance Use Care Coordinator helps connect those seeking help to necessary services such as counseling, chemical dependency, mental health services, employment assistance, support groups, primary care, rehabilitation, food assistance, and transportation.

The Road to Recovery does not need to be taken alone, and CHCNC is here to help.


Adults and children of all ages are welcome to utilize our services, which are offered throughout both St. Lawrence and Franklin counties.
