Community Heath Center of the North Country serves all patients, regardless of insurance status or ability to pay. Discounts for essential services are offered depending upon family/household size and income. You may apply for a discount at any one of our locations by filling out our Sliding Fee Scale application. The income guidelines can be found within the application, and a list of documents that will be needed to verify your eligibility can be found below. To obtain more information, please contact a care coordinator/case manager at the location where you normally receive your care.
Acceptable forms of Income Verification:
- Most recent Federal tax return
- IRS form W-2 or 1099
- One month of recent consecutive pay stubs
- Most current month of Social Security, disability, or pension benefit statements
- Documentation of other governmental or other monthly financial assistance
- Verification of student status
- Unemployment Benefits
