Our team has Care Coordinators located at each of our health center sites throughout both St. Lawrence and Franklin County. Care Coordinators can assist with all aspects of care and recovery, making sure that patients and their families are connected with the providers, programs and services they require for maintaining a healthy lifestyle.

Care Coordinators work with patients and their families to make sure they are receiving the care that they need. Care Coordinators can help with scheduling appointments, referrals, transportation, insurance enrollment, prescription assistance, and can also assist with housing and food.
