School Based Dental Home Program (SBDHP)

CHCNC also carries out the School Based Dental Home Program (SBDHP). This program provides an educational presentation, screenings, sealant placements and follow-ups for pre-k, 1st, 2nd and 3rd graders who have received permission from a parent or guardian. The program will travel to 22 schools in 17 districts.


Community Health Center of the North Country staff, Kim Halpin, Registered Dental Hygienist, and Lori Lamb, Dental Assistant, will be using mobile dental equipment to provide on-site services. A parental consent form providing information about the program and date is sent home with students at each school before the programs scheduled arrival.


Dental Sealants provide many oral health benefits to young children, including the prevention of tooth decay and preservation of the molars. If you would like more information about the program, please contact Kim at (315) 379-8207 or email her at sealant@chcnorthcountry.org.
